Needs Assessment

The first step in understanding how social factors impact health is to collect robust data on the situation in which people live and relate it to their health status. The Community Health Needs Assessment attempts to do that, using data from Mercy Care’s new Social Determinants of Health screening tool in the Electronic Health Record.
