Are you ready to get connected?
To use MyChart, you need:

- Access to the Internet: There are many public computer labs in the community. Our staff can refer you to one if needed.
- A private email address: If you do not have an email address, you can get one for free at www.gmail.com or www.yahoo.com.
- An activation code: Our office must provide you an activation code to access MyChart for the first time. If you are an established patient, your activation code is printed on the last page of your After Visit Summary, or you may call 678-843-8600 to have the link sent to you. If you are a new patient, ask your provider for the code when you visit a Mercy Care clinic.
- Your birth date and social security number: To protect your privacy, you will need to input your date of birth and social security number to sign up. If you do not have a social security number, call us at 678-843-8600 for help.
