Services are planned and delivered by a team that works together for your health. Everyone receives exceptional care, no matter your insurance or income status. On a daily basis, taking care of yourself is up to you, but we are here to help.

Primary Medical Care

Our team of medical providers is here to meet the acute, chronic, preventive and routine healthcare needs of adults 19 years of age and older. Primary care services fall into three categories:

Chronic Care

Mercy Care offers patients the knowledge, tools and resources to understand and manage their chronic conditions such as diabetes, high blood pressure, asthma, high cholesterol, obesity, and infectious diseases like HIV. Your doctor can offer advice, resources, and referrals to our many health education courses designed to help patients confidently take their treatment home with them.

Routine health screenings and early detection of problems can be the best way to fight and overcome serious chronic conditions.
We also provide screening for:

Pediatric Primary Care

Pediatric primary healthcare for children and adolescents is family centered. Care encompasses health supervision and guidance for parents; monitoring physical and psychosocial growth and development; age-appropriate screening; and diagnosis and treatment of acute and chronic disorders.

Behavioral Health and Psychiatry

Behavioral health clinicians work as a member of the overall primary care treatment team to provide the best integrated care possible. Medical patients are screened for concerns related to depression, anxiety, addiction and other behavioral health issues. When mental health concerns are identified, an immediate handoff to a licensed clinician can occur for further assessment and recommendations. Our behavioral health specialists also have a team of Mercy Care psychiatrists to whom they can make referrals.

Counseling and Medication

Behavioral counseling has a positive impact on patients’ management of chronic health conditions and leads to overall improved outcomes and health. Individual counseling and medication management is provided when needed. We also offer spiritual support through pastoral programs.

Substance use disorders

Patients dependent on alcohol and/or drugs are encouraged to meet with one of our certified addiction counselors for a full assessment and connection to ongoing care. Counselors offer addiction support groups and ongoing individual counseling. In addition, the team offers peer level support and encouragement with the help of a certified addiction recovery empowerment specialist.

Virtual visits

Most behavioral health visits can now take place over the phone or by virtual visit. Current patients can contact their behavioral health specialist to learn about options and scheduling. New patients can call 678-843-8600 and ask to talk to a member of the behavioral health team about scheduling options.

Case Management

Patients experiencing homelessness and who also have a mental illness that is not currently being treated are eligible for case management services. Case managers provide personalized support for people ready to get on the road to recovery, and they can link individuals directly to resources such as housing and financial help. Case managers usually work with an individual for a period of 90 days or more while focusing on the person’s goals and offering encouragement, support, and guidance along the way.

Dental Care

Routine dental care is available for adults and children at Mercy Care. Appointments are recommended, but emergency and walk-in patients can be seen at all sites on a first-come, first-served basis.

Vision Care

Mercy Care’s Chamblee and Decatur Street clinics are fully equipped to provide quality eye care services.

Thanks to partners and donors, some patients qualify for free prescription eyeglasses through Georgia Lions Lighthouse Foundation.

In addition to comprehensive eye exams, Mercy Care offers:

  • Annual dilated diabetes examinations
  • Diabetic retinopathy monitoring for at risk patients
  • Glaucoma diagnosis and treatment
  • Pediatric examination and treatment
  • Diagnosis and management of retinal diseases, such as macular degeneration

 

Health Education

Health education makes medical conditions and healthy living practices easier to understand. Education also empowers patients to improve their wellness and prevent chronic disease through community classes, group classes and individual appointments.

HIV Integrated Treatment

Testing and treatment for HIV is integrated into our primary medical services. If you want a free rapid HIV test or want information about affordable prevention with including PrEP for prevention call or text 678-843-8566.

Mercy Care’s Early Intervention Clinic (Ryan White Program Part A) delivers comprehensive HIV medical care including medical labs, medication, oral health, behavioral health. These services support those with low income, uninsured or underinsured living with HIV. The ultimate goal of HIV treatment is to achieve viral load suppression which means your body maintains low amounts of the virus in it, also known as being undetectable. Being undetectable keeps you healthy because the virus is less able to attack your immune system cells and reduces your ability to pass HIV to others. Eligibility is determined prior to enrollment and reassessed every 6 months. Those entering the early invention program receives a continuum of care on-site such as case management, housing, nutrient education, and medication assistance.

Breast and Cervical Cancer Program

Mercy Care helps low-income patients ages 18-64 get critical access to breast and cervical health education, clinical breast exams, screening mammograms, pelvic exams and PAP tests. We also provide patient navigation and diagnostic follow-up services including diagnostic mammograms, ultrasounds, and biopsies. Through a supportive team, the program successfully assists patients with overcoming barriers to care such as poverty, a lack of insurance, limited English proficiency, and low levels of education.

Outreach

Mercy Care’s Community Health Outreach Program (CHOP) seeks out chronically homeless people on the streets, in parks and shelters. They earn trust by being visible five days a week, providing hygiene kits, jackets, water or transit cards and then connecting people to services that can be both life-changing and life-saving.

A natural extension of CHOP is Street Medicine. On week days and nights, a psychiatrist, nurse practitioner, registered nurse, healthcare students and a case manager team up for Street Medicine. Together, they provide mental and physical healthcare including medications on the street, under bridges and in parking lots. The goal is to provide treatment that leads to wellness, housing and reconnecting those experiencing homelessness to their support groups such as family or church. Telemedicine often helps keep Street Medicine effectively staffed while on the street.

Enrollment Assistance

The federal government (45 C.F.R. § 155.215(a)(1)(iv)) requires that we tell you about:
– Any products we sell that is not health insurance
– Any present or previous employment relationships in the last five years with anyone who issues health insurance, stop loss insurance or companies who issue health insurance or stop loss insurance. This includes any current employment relationship between a spouse or domestic partner.
– Any non-prohibited financial, business, or contractual relationships that we have with a health insurance issuer or issuer of stop loss insurance or companies of health insurance issuers or issuers of stop loss insurance. This includes both existing and future financial, business, and contractual relationships.

 

Pastoral Care

Pastoral Care offers a service central to our core values in compassion and reverence for the dignity of every person. An important part of receiving treatment of any kind at Mercy Care is for a patient to feel seen and heard before, during, and after their appointments.

Our pastoral care team is trained in a variety of techniques to warmly interact with patients, including persons with trauma. By being present and talking to patients as they wait to see a provider, our pastoral care team provides comfort, lifts spirits and often affirms the deep faith they find in our patients.

 

 

Recuperative Care

Before Mercy Care started its 30-day Recuperative Care program in 2010, hospitals were challenged with how to avoid discharging the homeless back to the street or to shelters. Without a safe place to heal, they would often end up back in the hospital’s ER. Mercy Care’s Recuperative Care staff provides healthy meals and a safe, clean, temporary home in which to recover located at Gateway Center (men) or City of Refuge (women). By facilitating appointments with health providers and ensuring patients take and tolerate medication, Mercy Care’s team catches potential complications quickly. They also provide education and help with mental health or addiction issues. As a result, 70% of Recuperative Care patients are discharged to the care of family or housing.
