What to know before your visit.
Making an Appointment
You may make an appointment by calling (678) 843-8600. If you have one, please bring your insurance card to every appointment. We will collect co-payment according to your health insurance plan or the sliding fee scale before you see a caregiver. Walk-ins are welcome but wait-times to see a provider may be longer and is based on provider availability.
As a Federally Qualified Health Center, Mercy Care is required to collect information from those applying for discounted fees and copays during the registration process. First-time patients needing the discounted sliding fee scale based on income MUST bring documents specified below with them before they can be seen.
A day or two before your visit, Mercy Care will provide a reminder call. If you cannot keep your appointment, please call us at (678) 843-8600 as soon as possible – no later than 24 hours before your visit. We will help you to reschedule.
You may also schedule your appointment through MyChart. Once you have set up your MyChart account, you can even make or change appointments through the MyChart app.
Changing Your Appointment
Please arrive 20 minutes before your appointment to check in and complete your registration.
If you can’t make it to your appointment on time, call (678) 843-8600 to see if you can still come in or if you need to reschedule. You may also reschedule your appointment through MyChart. If you arrive up to 10 minutes late for your appointment, a clinician may be able to see you, but your time may be shortened so other patients are not kept waiting.
If you know you will be unable to keep a scheduled appointment, contact us at least 24 hours in advance to reschedule so we may use that time for others.
Paying for Your Visit
Mercy Care provides clinically excellent healthcare regardless of your ability to pay. If you would like to apply for sliding fee-scale discounts, the federal government requires proof of household income. This is a requirement for first-time patients and those who haven’t visited Mercy Care in the past 12 months.
For your first visit or annual return visit, please bring:
- Photo ID (if applicable)
- Proof of current address in the form of a utility bill, lease agreement, or homeless shelter letter. If your ID has your current address on it, additional proof of address is not required.
- Proof of income – Bring one of the following as proof of household income only if you want to apply for discounted fees/copays based on a sliding fee scale.
- A minimum of two (2) paycheck stubs, no older than 30 days.
- If you currently are not receiving income, you must present a Wage Inquiry form from the Georgia Department of Labor, no more than 30 days old.
- If you are self-employed, you may choose to show a recent tax return.
- If you are paid in cash, you will need to provide a letter from your employer stating your rate of hourly pay, amount paid each pay period and how often you receive payment.
If you have commercial insurance, Medicaid, or Medicare, you do not need to bring proof of income. You need only your current insurance card and photo ID – unless you need discounted co-pays. Then the above is needed.

Pago de su visita
Los pacientes que soliciten tarifas o copagos con descuento y cumplan con los criterios de nuestra escala de tarifas variables, basados en comprobantes de ingresos, pueden pagar tan solo $35 por visita. Los pacientes que presenten comprobantes de no tener ingresos y las personas que estén experimentando situación de calle pueden no recibir cargos por los servicios.
Escala de Tarifas Variables y Documentos Requeridos
Para aquellas personas sin seguro médico, o cualquier persona que solicite tarifas o copagos con descuento, se requieren los siguientes documentos para aplicar a la escala de tarifas variables; estamos obligados a recopilar esta documentación durante el registro.
Pedimos que los pacientes que necesitan tarifas con descuento y que acuden por primera vez traigan los documentos que se enumeran a continuación. La información que usted proporcione determinará su costo en el momento de su cita. Revisaremos estos documentos anualmente o cada vez que su información cambie.
Si se registra por teléfono, recopilaremos sus datos de registro durante la llamada; solo necesitará mostrar la documentación cuando llegue a una de nuestras clínicas.
Por favor traiga uno de cada uno de los siguientes:
Identificación con fotografía
- Licencia de Conducir de Georgia (GA) o Identificación con Foto de GA vigente
- Identificación de trabajo con fotografía y nombre de la empresa
- Pasaporte
Comprobante de domicilio
- Factura de servicios públicos: electricidad, cable, agua
- Contrato de arrendamiento
- Carta de un Programa de Asistencia del Estado
- Si su identificación muestra su dirección actual, no se necesita comprobante adicional.
Tarjeta de seguro (si corresponde)
Aceptamos la mayoría de los planes de seguro principales.
- Seguro comercial: Tarjeta de seguro más reciente
- Medicaid: Tarjeta de Medicaid más reciente (Peach State, PeachCare for Kids, Wellcare, GHP o Amerigroup)
- Medicare: Tarjeta roja, blanca y azul o su tarjeta de Medicare HMO
Comprobante de ingresos
Presente uno de los siguientes solo si desea solicitar tarifas/copagos con descuento:
- Al menos dos recibos de pago con una antigüedad no mayor a 30 días
- Si actualmente no recibe ingresos: un formulario de Consulta de Salarios del Departamento de Trabajo de Georgia con fecha dentro de los últimos 30 días
- Si es trabajador por cuenta propia: una declaración de impuestos reciente
- Si recibe pago en efectivo: una carta de su empleador que indique su salario por hora, el monto pagado por período de pago y la frecuencia de pago
Service Dogs
The Americans with Disabilities Act (ADA) allows service dogs to accompany you during your visit. Service dogs must be trained to your command and housebroken.
Please note: The ACA does not recognize therapy, emotional support, comfort or companion dogs and other pets are the same as it does service dogs. Animals other than service dogs are not permitted in Mercy Care buildings.
