Mercy Care Community Outreach

Mercy Care’s Community Health Outreach Program (CHOP) meets people where they are — on the streets, in parks, and in shelters — building trust through daily outreach. The team provides essentials like hygiene kits, water, jackets, and transit cards while connecting people to life-saving services.

Street Medicine is an extension of this work. A psychiatrist, nurse practitioner, RN, students, and a case manager travel through the city day and night, offering medical and mental health care — including medications — in encampments, under bridges, and in parking lots. The goal: support wellness, stabilize health, and reconnect people experiencing homelessness to housing, family, and community. Telemedicine helps the team stay fully staffed and responsive in the field.
